Your provider refers you

Your physician, PA, or NP identifies coaching as a complement to your clinical care and refers you to Stratum.

A no-obligation 20-minute call to answer your questions and confirm coaching is the right fit.

Your 90-minute intake session establishes where you are across all four pillars — and leaves you with a 30-day action plan in hand.

Biweekly coaching sessions, accountability, and real-time adjustment over three months.

Who we work with

Coaching for patients managing chronic lifestyle-related conditions

Stratum works with patients who have a diagnosed condition where lifestyle modification is a recognized component of treatment or disease management — some examples include hypertension, pre-diabetes, obesity, metabolic syndrome, and similar chronic conditions. If your provider believes you could benefit from structured behavioral support, you likely qualify.

Coaching is not a substitute for medical care. Stratum works alongside your provider — not as a substitute for them.

You complete the EQ-i 2.0 assessment online before your intake session. Takes about 20 minutes. Your coach interprets the results in advance.

Completed before your intake appointment (~30 min). Scored and interpreted by your coach in advance so your EQ profile informs your action plan from session one.

SESSION 1: Intake, baseline assessment & action plan — 90 min

Full lifestyle baseline across all four physical pillars combined with your EQ-i 2.0 debrief. You leave with both your EQ profile and a personalized 30-day action plan in hand.

SESSION 2: First accountability check-in — 50 min

Real-world debrief on your first month. What worked, what didn’t, what surprised you. We adjust and move forward with real data.

SESSION 3: 30-day review & deepening — 50 min

Structured progress review against baseline. Reinforce what’s working, troubleshoot barriers, deepen your highest-priority habit area.

Month 2 - Momentum

SESSION 4: EQ integration — habits and emotional patterns — 50 min

A focused session connecting your EQ profile to your specific behavior change challenges. Leverage your emotional strengths to accelerate your health goals.

SESSION 5: Pillar focus & habit building — 50 min

Dedicated work on your highest-priority habit area. This is where change starts to feel less effortful and more natural.

SESSION 6: Mid-program milestone — 50 min

60-day review. Celebrate progress, lock in momentum, and set the agenda for the final stretch.

SESSION 7: Sustainability planning — 50 min

Build the habits and systems — physical and emotional — that keep you on track after coaching ends.

Includes one 50-minute session per month plus ongoing between-session support via phone, text, or Marco Polo video messaging for questions as they come up. No long-term commitment required. I am an experienced educator with nearly two decades of experience in physician assistant (PA) education and a doctoral degree in health science. As a National Board-Certified Health & Wellness Coach, Precision Nutrition Level 1 coach, and certified Emotional Intelligence consultant, I bring a depth of clinical literacy that most wellness coaches simply don’t have.

When you meet with me, I have a solid understanding of your diagnosis, I understand what your provider is trying to accomplish, and I know how to translate clinical goals into the kind of daily habits that actually change your health — not just your intentions.

Health coaching at Stratum is not generic advice. It is an individualized, evidence-informed engagement built around your life, your condition, your emotional intelligence profile, and your goals.

Making coaching accessible

Use your HSA or FSA to pay for coaching.

The IRS permits HSA and FSA funds to be used for services that treat, mitigate, or prevent a diagnosed medical condition. With a Letter of Medical Necessity (LMN) from your referring provider, Stratum coaching fees qualify. Here’s how it works:

Get referred

Your physician, PA, or NP refers you and writes a Letter of Medical Necessity documenting your diagnosis and the clinical rationale for coaching.

Enroll in coaching

Complete your intake and sign your client agreement. Your LMN stays with you — no paperwork from your provider’s office is required beyond that.

Pay with HSA/FSA

Use your HSA debit card or submit for FSA reimbursement. Stratum provides itemized receipts for every session.

Start coaching

Your first session — intake, baseline, and 30-day action plan — is scheduled and you’re on your way.

What referring looks like

Identify a qualifying patient · mention Stratum and provide this website or the referral card · sign a one-page Letter of Medical Necessity using the provided template if the patient wishes to use HSA/FSA funds. That’s it. No paperwork, no clinical coordination required. Your patient schedules directly with me.

Not referred by a provider? Stratum also works with individuals pursuing general health and wellness goals outside of a clinical referral.
